Dr. David Perlmutter, a distinguished neurologist and New York Times bestselling author, joined John Miles on the Passion Struck podcast to discuss the groundbreaking insights from his new book, Brain Defendres. The conversation delves into the critical role of the brain's immune system, specifically microglial cells, in determining our cognitive future. Dr. Perlmutter emphasizes that brain health isn't just a concern for old age but is shaped by lifestyle choices made decades earlier.

The Brain's Immune System: A Double-Edged Sword

Dr. Perlmutter introduced the concept of microglial cells, the brain's immune cells, which constitute about 10% of the brain's cellular makeup. These cells are not merely reactive to trauma or infection; they are constantly in surveillance mode, nurturing neurons, synapses, and the blood-brain barrier. However, they possess a dual nature, capable of shifting from supportive 'brain defenders' to destructive 'evil twins.' This shift, Dr. Perlmutter explains, is fundamentally driven by changes in their metabolism, which mirrors the body's overall metabolic health.

"When our metabolism is doing what it should do — keeping our blood sugar in check, our weight in check, our blood pressure where it needs to be, reducing inflammation — then these brain immune cells are supportive," Dr. Perlmutter stated. "But they can shift on us. They can turn their backs and become what I call in the book, the evil twin... when they are challenged by inflammation or metabolism issues in the body, and now they are destructive."

This fundamental underpinning, he argues, applies not just to Alzheimer's but to all neurodegenerative conditions, including Parkinson's, MS, PTSD, and even long COVID. The pivotal role of these cells means that understanding and influencing their function is key to charting a better brain destiny.
