In this episode of The Drive, Peter Attia sits down with Dr. Michael Davidson, a distinguished cardiologist and lipidologist, to delve into the latest advancements and ongoing challenges in preventing cardiovascular disease (CVD) and Alzheimer's disease. Dr. Davidson, the founding CEO of New Amsterdam Pharma, shares his extensive expertise, highlighting the critical role of early and aggressive LDL-C lowering, the complex interplay of various lipid markers, and the exciting prospects of novel therapeutic agents.
The Imperative of Early LDL-C Lowering
Dr. Davidson opens the discussion by emphasizing the profound impact of early and sustained LDL-C lowering on preventing atherosclerotic cardiovascular disease (ASCVD). He introduces the concept of the "8-gram rule," which posits that accumulating 8 grams of cholesterol in one's lifetime leads to heart disease. This can be achieved, for example, by maintaining an LDL-C level of 200 mg/dL for 40 years or 100 mg/dL for 80 years. Conversely, keeping LDL-C below 80 mg/dL throughout life significantly reduces the risk of heart disease. Dr. Davidson's personal experience, losing his father to a heart attack at age 47 and witnessing his brother undergo bypass surgery at 44 despite a strict diet, underscores his passion for early intervention. He argues that treating LDL-C should be akin to managing blood pressure or smoking cessation, where intervention is initiated regardless of immediate risk, due to its causal role in disease progression. He expresses frustration that this proactive approach is not more widely adopted, attributing it partly to the misconception that lifestyle changes alone can adequately manage LDL-C, and to outdated primary prevention guidelines that do not sufficiently emphasize early intervention.
